Our Purpose
“The mission of Alternated Living Considerations (ALC) is to provide the residents of Sun City Roseville with a planning tool for a meaningful alternative when it becomes clear that a different living situation is necessary.”
In response to requests by residents of Sun City Roseville, the Sun City Roseville Foundation compiled the following information to assist our residents in planning for their future. It was not our objective to recommend a particular agency, service or community, but rather to provide a tool for our neighbors and friends. In compiling the information on the following pages, we express our appreciation to the various agencies and communities for their participation and cooperation on this project. We have intentionally omitted fee structures for the various services and residential communities as listing those prices would involve constant monitoring and updating of the data. Understandably, we leave this to the various organizations that we identify. Additionally, the fees for residential communities vary widely based upon your personal needs, wishes and requirements. Most communities include a one-time, up front, “Community Fee” in addition to the monthly fees. Contact information is listed at the top of the information pages.
You must consider what lifestyle you wish to maintain. How is your current health? Do you wish to remain in the same general area or move to another city or state? If you are going to move to another city, are you comfortable with changing doctors? Do you think you might want to remain in your home or move to a multi-care facility? If you currently have in-home care, can you afford to increase that level of care at home? These are just a few of the questions you need to consider.
It is especially important to develop a plan before you are reacting to a crisis! You should be aware of the cost of ‘future’ care needs if you decide to relocate to your new residence. Can that residence handle your needs if they increase? Over the next few pages, you will find information to help you determine where to begin this process.
Getting Started
Most of us will reach a point when some consideration should be given to alternative living arrangements. At issue is whether one wishes to be in control of the transition or be forced to make a quick decision during a crisis. The following questions should be considered before you determine what facilities, services, health care and amenities will be needed following such a change.

FREE ADVISORY SERVICES FEE BASED SERVICES

Free Advisory Services
The organizations listed below can help you to assess what type of assistance you may need. They can educate you on the various lifestyle options that most closely fit your unique needs. If you choose to remain in your home, they can refer you to licensed and bonded in-home care agencies. If after a consultation you would like to consider relocation, some of the organizations listed below will offer to take you on a tour of prospective living communities. There is no fee to the client for the evaluation and tour. If you do select your preferred community, the agencies below will receive a referral fee from that community. You just need to start the process by making that phone call….
Professional/For Profit Agencies
If you have determined you would like to remain in your current home with the assistance of a caregiver, the fee-based agencies below can help you to find in-home assistance.
There are at least 3 ways you can hire a caregiver to assist you.

You can hire them privately without the assistance of an agency. In this case, you are responsible for payroll taxes, social security, Medicare and unemployment. You are responsible for background checks.

You could use a Full-Service agency. They do all the background checks and handle payroll & taxes.
-
You could use a Domestic Referral Agency (DRA). In this case, the agency recruits a caregiver for you. However, the caregiver is not an employee of the agency. You may still be responsible for taxes, workers compensation and liability insurance.
The agencies listed below fall into the Full Service or Domestic Referral bracket. It is your responsibility to interview them to determine which category they fit into.
Multi-Level Care Facilities
We are fortunate to have many residential communities nearby. They are divided into several categories based upon the level(s) of care they can provide to their residents. The major levels of care are broken down as follows:
Independent Living = The resident is fully capable of caring for themselves. However, some communities offer amenities such as meals, housekeeping and limited transportation.
Assisted Living = The resident requires meal preparation and some level of assistance with the activities of daily living and medication management.
Memory Care = The resident requires assistance with all activities of daily living. Additional security is provided to maintain the safety of the residents.
Continuous Care Retirement Communities (CCRC) = These communities offer all levels of care (including skilled nursing services) in one community. Their goal is to allow the resident to remain in the same community for the remainder of their life.
Additional levels of care can also be found in some of the following facilities.
Skilled Nursing Facilities = 24-hour nursing care is provided along with physical, speech, respiratory or occupational therapy. Pain management, physical rehabilitation and intravenous services are provided in this environment.
Respite Care = Primarily short-term care. Some facilities offer the opportunity to stay for a day, a week or even a month. This short stay allows a resident to experience the lifestyle while giving their caregiver a much-needed break from the responsibilities of caring for their loved one.
On the following pages, you will find a listing of several nearby communities. We have listed their contact numbers and levels of care provided. We have also listed additional information which you may find pertinent. As fees vary over time, we have not listed the cost structure for each facility. That information can be collected by contacting and touring the communities. You will find they are all pleased to take you on a tour of their facilities and discuss the fee structure. However, it is recommended that you call first and make an appointment.
Multi-Level Care Facilities
Know Before You Go
BEFORE you begin touring, there are some basic questions you might want to inquire about. There are many senior communities in the area, and you want to find the one that fits you just right. Plan on inquiring about what makes each facility unique or different from the others. Look for the following signs:

First impression: Are things bright and cheery or old and worn? Does the open space and entrance look inviting? Is this a more “formal” atmosphere or a homey “comfortable” feel? What appeals to you?

Observe the staff: Do they look happy? Are they interacting with residents?
-
Observe the residents: Are there many residents moving about the community? Do they look happy and engaged in the community? If possible, strike up a conversation with one or more and ask them about their experiences.

Overall, how does the entire community look? Is it kept up well? If there are outdoor spaces, are they being maintained accordingly?

When setting up an appointment, you might inquire if you can work it around a meal and be able to taste the food.
Keep in mind several questions that you may wish to ask:

What types of residents does this community serve? Is this an Independent Living community or an Assisted Living Community? Memory Care? Does it offer “aging in place” so you do not have to switch apartments should something happen to your physical capacity?

What are the amenities included in your monthly fee? Things such as housekeeping, meals, social activities, outings, etc.a. Ask about the services you may be required to pay extra for such as meal delivery, medication administrations, etc. (**This is an important question as there are many services that can be provided a la carte. Many residences are moving toward a monthly “Level of Care (LOC)” basis depending on the needs of each resident.)

What is the staff-to-resident ratio? Are there enough staff on site to assist if needed?

How many meals a day are included and what are the operating hours offered? Can meals be taken to your room?

What will the staff do to ensure an easy transition into the community?

Is transportation provided for medical appointments or shopping trips?

Will the family be supported through this process and be informed about how a loved one is doing?

Respite care provides the option of short-term support for adults and their caregivers. Stays may be full day (overnight) with weekly and monthly schedules. While allowing a welcome relief to the caregivers, respite care also offers the guest to experience the services and amenities provided at this facility.

Geriatric Care Managers
A Geriatric Care Manager is a professional educated in various fields of human services:

They have been trained to assess, plan, coordinate, monitor and provide services for the elderly and their families. A Geriatric Care Manager acts as an advocate for the older adults. As a member of the National Association of Professional Geriatric Care Managers, they are certified by the National Association of Social Workers, the National Academy of Certified Care Managers or the Commission for Case Managers. Geriatric Care Managers will provide an initial assessment of your situation on a fee basis determined by the region and firm. Continuing support is on an hourly basis. They should be available by phone 7 days a week and 24 hours a day. You should also be able to contact the other members of their team (nurses or support staff) in the event you can’t reach your Care Manager.
An example of a situation where a Geriatric Care Manager might be helpful would be when the family lives a great distance away from their parents. They would be able to assess the parent’s living situation, coordinate in home aids or in some cases assist with the coordination of moving the parent closer to the family or into one of the various Multi Level Care Facilities.
A Fiduciary
A Fiduciary is a legal or ethical relationship of trust between two or more parties. Typically, a Fiduciary prudently takes care of money for another person. One party, for example a corporate trust company or the trust department of a bank, acts in a Fiduciary capacity to the other one, who for example has entrusted funds to the Fiduciary for safekeeping or investment. Likewise, asset managers—including managers of pension plans, endowments and other tax-exempt assets—are considered Fiduciaries under applicable statutes and laws.[Note 1] In a Fiduciary relationship, one person, in a position of vulnerability, justifiably vests confidence, good, reliance, and trust in another whose aid, advice or protection is sought in some matter. U. S. Senior Vets is a non-profit 501(c)3 that provides a concise explanation of the Non-Service Connected Disability Pension. This is accomplished by using a network of volunteer veteran advocates, who provide no cost appointments and educational workshops on how this benefit offsets the cost of assisted living, skilled nursing or home bound care.
Funding is provided solely by corporate and personal tax deductible donations. The U. S. Department of Veterans Affairs has a tax free pension available to help qualified veterans, spouses and their surviving spouses pay for home health care, assistance living or nursing home care.
The Department of Veterans Affairs (VA) operates through state-based regional offices where applications are reviewed before being sent to the national offices. These local agencies are known as Veterans Service Organizations (VSO). Their officers are accredited by the VA but not employees of the VA. For that reason, you are advised to interview your local VSO carefully regarding their training, experience, success rate and filings per month before enlisting their assistance with establishing your qualifications and filling out your application.

What is Medi-Cal?
Medi-Cal offers free or low-cost health coverage for California residents who meet eligibility requirements. Most applicants who apply through Covered California and enroll in Medi-Cal will receive care through managed health plans. Medi-Cal has always covered low-income children, pregnant women and families. On January 1, 2014, California expanded Medi-Cal eligibility to include low-income adults. When you complete a Covered California application, your eligibility for Medi-Cal will automatically be determined. You can apply for Medi-Cal benefits regardless of your sex, race, religion, color, national origin, sexual orientation, marital status, age, disability, or veteran status. There are only a few facilities in our region that will accept residents under Medi-Cal coverage.
